An employee was manually lifting/flipping a large steel plate when he slipped on some grinding dust and lost his footing. The steel plate, which measured approximately 25" X 28" X 2.5" fell, crushing the employee's right hand. He sustained amputations to his index and middle fingers and was hospitalized.
